According to UFCOM policy, when students need to miss class for medical appointments:
Students must submit their request for an excused absence to the relevant course/clerkship director(s). Upon approval, the director will notify the student of makeup requirements and due date, if appropriate. Additionally, students will not be penalized for absence from class or other scheduled academic activities for medical reasons. This applies to absences for acute illnesses as well as to absences due to regularly scheduled ongoing treatment for dental, mental or physical health. For any questions and/or concerns regarding this policy, students are to consult the Associate Dean for Medical Education or Student Affairs.
Sample email to request an excused absence (Note: You do not have to divulge your medical history unless you want to do so):
Subject: Request for excused absence
Dear Dr. _____________,
My name is _____________ and I am really looking forward to being in your course/clerkship this coming ______________. I am writing to consult you about a dilemma. I have a standing health care appointment every Tuesday from 3-4 pm. Although I tried to reschedule this appointment, unfortunately, that was not possible. Therefore, I am requesting your permission for an excused absence on those dates and times. If permitted, please let me know how I can make up the time and/or assignments missed. My education is as important to me as my health, and I want to minimize the impact of my appointment. Thank you so much for your consideration of this request.
Respectfully,
_____________
